pi-setup gained a git_create_pr tool
Added a proper git_create_pr tool to pi-setup, with tests covering the PR state classification (new vs. update) so the branching logic is proven rather than guessed. The git-create-pr skill now just delegates to the tool instead of hand-rolling the push-and-open dance every time.
